To facilitate your pre-procedure care, it is important that you complete your medical history.  To expedite this process, it is essential that you have with you your list of medications, allergy information, physician information and any implanted electronic device information.  If you are a returning patient, please review your previously submitted medical history and re-submit the form after making any necessary changes.

To start your health history, we ask you to create a user name and password.  Please retain your user name and password for future visits to our facility.
